Posted Instructions for Using Photos

Does anyone have a boilerplate disclaimer and/or instructions for someone to obtain your permission to use your photo(s) on their websites that I could borrow?

I hate to do it because *I* don't like them, but if I find another one of my photos that someone has plucked from an image search and tried to pass off as their own, I'm going to have to start watermarking my photos.  I'm thinking the instructions could serve as an interim step

When I send images to pilots, I usually include a word document declaring that the bearer of the file(s) has my permission to print the images for "Private, Non-Commercial Use Only" and to contact me if there is an issue.

Not sure how just having the language on your site will keep sticky mice from playing though.

Thanks, Jay.  Yes, I know it won't stop them all, but hopefully it'd make some pause and think about it.  I already have all of my photos on Flickr set to 'all rights reserved'.  I just don't want to start watermarking all of my photos... but I will if my shots keep popping up on Blogger.
